Advance Directives
Advance Directives are legal documents that help you plan for your future medical care.
A Medical Power of Attorney allows you to choose the person who will make medical decisions for you when and if you cannot.
A Directive to Physician, also called a Living Will, helps you plan for end-of-life care.
We have staff available to discuss more about Advance Directives. Please ask to speak to our Medico-Legal Partnership Department
