Bring a government-issued ID, proof of income or household size, and insurance card(s) if applicable. Medical records or medication lists are optional but recommended.

How To Become A Patient

New patients are welcome at New Horizon Health Center in Brownsville, Texas. To become a patient, simply visit our enrollment office at 91 E. Price Rd., Brownsville, TX 75821 or call 956‑548‑7400 to schedule an appointment. Our team will guide you through registration, help you complete necessary forms, and explain which services are available to you. Please read below on what you need to bring before your appointment.

What You Need To Bring

To register as a patient, please bring:
- A government-issued ID (driver’s license, passport, or state ID)
- Proof of income or household size (for sliding fee scale eligibility)
- Insurance card(s), if applicable
- Any medical records or medication lists (optional but helpful)

Sliding Fee Scale and Discounts, and How To Qualify

New Horizon Health Center provides care to all patients, regardless of insurance. Our sliding fee discount program adjusts costs based on income and family size, making quality healthcare accessible for the entire community. New Horizon Health Center utilizes the Federal Poverty Guidelines (FPG) to offer Fixed sliding fee discounts for medical and dental services. Fixed discounts are based on income and number of dependents in the household.
Patients with or without insurance coverage may apply for a Fixed Sliding Fee Discount for services.
Eligibility is only valid for one year. If the eligibility information is not updated within 30 days from the date of service, your account will be charged at 100% of services.

Bring proof of Identification with a picture ID which can be in the form of:
- Current Driver’s License
- TX Identification Card
- Voter Registration Card
- Passport
- Consulate ID Card
- Legal Resident documents
If you do not have a picture ID please bring 2 of the following:
- Birth Certificate
- Social Security Card
- School Record
- Voter Registration Card
- Marriage License
For children, please bring 1 of the following:
- Birth Certificate
- Verification of Birth Facts from hospital
Proof of Income (as applicable)
- Earned Income:
  - Copy of most recent four paychecks stubs.
  - Verification of Employment or Statement from employer with company’s information.
- Unearned Income:
  - Recent TANF letter from Texas Department of Human Services
  - Social Security Administration and/or Social Security Supplement
  - Child Support award letter from Attorney General
  - Statement of Support with ID of person writing statement
  - Retirement Pension reward letter
  - Veteran's pension letter
  - Unemployment award letter from Texas Workforce Commission
- Self-Employed (bring one of the following):
  - Self-Employment declaration forms/letters from the last 2 months
  - 1040 Income Tax Forms with W2 or 1099
